Available Funding Options
Home Care Packages (HCP) / Support at Home
Designed for older Australians needing coordinated in-home care. Eligibility is assessed by My Aged Care, based on your age and support needs.
National Disability Insurance Scheme (NDIS)
For people under 65 with a permanent, significant disability. You must be an Australian citizen, permanent resident, or hold a Protected Special Category Visa.
Transport Accident Commission (TAC)
Available for Victorians injured in transport accidents. Eligibility is determined by TAC, based on accident details and medical assessments.
Department of Veterans' Affairs (DVA) Community Nursing
Supports eligible veterans and war widows/widowers with a Gold or White Card, offering in-home nursing and personal care.
Commonwealth Home Support Programme (CHSP)
Provides entry-level help with daily tasks for older people. Eligibility is assessed by age and support needs.
Transition Care Programme (TCP)
Offers short-term care after a hospital stay to help older individuals regain independence. You’ll need an assessment and recent hospital discharge.
Post-Acute Care (PAC)
Short-term services to help after a public hospital stay. Eligibility is determined by your hospital team, based on recovery needs.
